How to use a Comprehensive Medical Indemnity Hospital Surgical Plan
1- On the back of your insurance card look for the PPO Network that is affiliated with your plan (i.e. Aetna, First Health, etc.) this is the Network that your medical bills will be sent to for payment from your doctor, medical provider or hospital. Remember there are more than 1.5 million providers available to you in your national PPO network
2- Although not necessary, stay in your PPO when possible, don't worry if your provider isn't in the PPO network your plan will still pay your medical bills
Comprehensive Coverages paid by an Elite Indemnity Medical Plan
1- The plan pays $3,000 for the first day in any year that you are admitted to a hospital
2- The plan pays $6,000 every day that you are in the hospital, this amount increase by 25% up to $12,000 per night at year 5 FACT: The sticker price or charge master price in the US per day in the hospital is $3,000 before adjustments
*Note: If the sticker price charge is reduced from $3,000 to $1,500 the indemnity plan has to pay you the remaining $4,500
Hospital Observation Benefits

Surgery Benefit
$50,000 per year three times the scheduled amount for inpatient and outpatient
"3x's the scheduled amount of surgery" means that the gross billed charge (the sticker price) submitted by the hospital or surgeon is three times higher than the insurance plan's official fee schedule
*The total sticker price billed to your insurance company for a standard, mid-range surgery (such as a gallbladder removal or hernia repair) generally ranges between $7,000 and $20,000.
FACT: The average PPO discount from the First Health Network typically ranges from 30% to 50% off the standard provider-billed charges
Ambulatory Surgical Benefit
The ambulatory surgical center (facility) benefit pays a fixed rate of $3,000 per day. This amount is paid for outpatient facility charges in addition to the separate surgical schedule and anesthesia benefits
*Daily Assistant Surgeon Benefit pays 20% of eligible surgical benefit. Fact: An assistant surgeon usually receives 16% of the primary surgeon's eligible surgical benefit or allowable amount
*Daily Anesthesiologist Benefit pays 25% of the eligible surgical benefit. Fact: For an average, routine surgical procedure lasting 1 to 2 hours, an anesthesiologist makes an estimated $300 to $800 in personal revenue
Doctor's Office Visit with Rollover
Elite Plus Plan pays $200 for 10 doctor office visits per year. The rollover provision allows a five-visit carryover per policy year.
Fact: High cost-of-living areas push routine visits toward the higher end, averaging around $137 to $200 for basic care.
